OK so I’ll try to make this short. I have really bad anxiety, panic disorder, and attention deficit disorder. I started using crystal meth primarily for weight loss about 6 months ago. At first it was excellent because I lost weight, but also it helped me be really productive at work, i stopped procrastinatig for everything, and I was able to accomplish more. I never felt “high” just more energetic, less tired, and more productive. Then my face started caving in and I started looking like a crackhead and eventually started getting a little crazy — I thought the IRS was following me and I attempted suicide twice. I left the hospital AMA with my previous psychitrist so he then discharged me from his medical care. I quit my job and things are horrible but I decided I wanted to go back to school. I’m 25 very smart but just can’t seem to focus on one task. I heard Adderall works wonders but a lot of doctors frown upon people with drug abuse history or addictions. I really want to graduate college so I can have a few more options available to me and to make my parents proud. Plus i’m WAY too smart not too. I really would like an Adderall prescription. What things should I tell my psychiatrist to help get an Adderall prescription. By the way I heard it is an obesity medicine as well and since I am addicted to food and cant stop eating. It’s hard for me to buy groceries because i eat it all in like 2 days. I am rapidly gaining the weight back and exercise doesnt seem to work because i exercise just so i can eat more. Ive tried therapy and nothing seems to work. My social anxiety disorder was probably the number one issue that I was dealing with but now my ADHD is since I am starting school soon and I have no idea how I will be able to study, listen to lectures, read text books. I can’t even focus on this it was so hard to write this. HELP!

I can tell you are having a hard time writing this.
And so, list these things to tell your new psychiatrist:
1) History of ADHD, unmedicated
2) History of suicide
3) History of delusional ideation (The IRS is following you…)
4) History of compulsive eating.
5) History of social anxiety
6) History of self-medication and drug abuse
7) History of inability to focus and follow through

This is what you tell him and he will ask you about each of these in detail and you can elaborate about each one.
Then, and most important, ask about number 8

8) Desire for medical workup and medication.

Write these 8 points out and hand the paper to him as you enter. This will help him focus on healing you immediately with your problems instead of beating around the bush.

But above all, don’t be non compliant with treatment again. That is what got you discharged from your first psychiatrist’s practice.
