we know that they have side effects, so when we think the “total effect” of it, is it negative or positive?

I disagree. Antidepressants would be competely banned from the market if total balance of effects would be negative.

I would say positive. Dry mouth for example, or constipation risk is lower than a suicide risk when you balance it.
