If a person is an alcoholic, a drug addict, or has an eating disorder, there are in-hospital programs which insurance covers.
A smoker can still function, until they become disabled with lung cancer, emphysema, etc. That doesn’t answer the question as to why it is not medically treated as an addiction.

because smoking doesn’t interfere with your life the way drugs and alcohol do. Of course it stinks, it’s unhealthy, and some people don’t approve, but you can totally function in daily life while being addicted to smoking.
