Medical transcription: What is wrong with the following sentences?

Please only answer this if you are a medical transcriptionist or understand medical transcription. What is wrong with the following sentences?

1. She continued to have pain and footdrop despite having adequate pulse and signs of excellent profusion. There was a good doppler signal.

2. By manual exam revealed no palpable mass or adnexal enlargment.

3. He reported melanotic stools.

4. The patient was placed in the dorsolithotomy position.

5. The patient had normal active bowel sounds on examination.

6. Tyenol Number Three was ordered PRN pain.

7. Thank you Dr. Smith for your kind referral of this patient.

8. The scar on his left forearm measured approximately 2-1/2 cm.

9. The patient has a history of elicit drug abuse, however, has been attending rehabilitation for approximately one years time.

Don’t worry I’m not in school looking for answers, I already graduated in medical transcription, but I am still new to this field and am trying to get extra practice, so I have been working on some practice examples, but these ones I’m a bit stuck on! Any help would be appreciated!
