How many people maintain their shape thanks in part to substance abuse or unhealthy habits?

I work out regularly and try to watch what I eat, and although I’m in pretty good shape, I’d love to drop 10 pounds. Unfortunately, it seems like diet and exercize just aren’t enough to maintain an excellent figure, especially at my age. Of course when you’re already in pretty good shape it’s much harder to lose weight than when you weigh 250 pounds. Yet I envy those with ultra slim bodies in spite of wondering whether they’re really any healthier than I am.

For example, I know a number of women who have that sleek physique I’m striving for who don’t really work out or watch what they eat but who seem to maintain their thin figure thanks to cigarette addiction or some sort of substance abuse. Likewise, many of the guys with the hardest abs and the largest biceps use steroids. (Not that I’m necessarily attracted to that Michelin Man muscle-bound look.) This makes me wonder what percentage of the beautiful people, especially those over 25/30, are really all that healthy. Any thoughts?
