in the “after” pics, a lot of them are shown to have sores and scabs on their faces. Is this from lack of hygiene or does the drug actually cause it?

The drugs make you itchy and you scratch and that too becomes addictive.

It’s probably caused by the drug, and greatly exacerbated by an generally unhygienic and unhealthy lifestyle.
