has anyone who appeared on the show “Intervention” die because of their addiction? if so, who?

Hmm, I’ve never seen them report at the end of an episode that a person has died. I’ve seen it state a person has relapsed. I guess if they followed the people for some time (months or years), someone on the show may OD eventually.
