How would one go about finding a physician that would prescribe and monitor medication used for crack cocaine dependency.

You need to be inpatient for crack addiction. You can’t just have a doctor give you meds to detox at home. You need a chance to get away from the drug and being at home is too tempting to go back out and get some.

Contact a rehab to be admitted.
